By signing this document, I understand my participation in Yoga classes includes, but is not limited to, participation in meditation techniques, yogic breathing techniques, and performing various Yoga postures and body movements. I hereby acknowledge and agree to enroll in this yoga program at my own will. I am participating in the yoga classes offered by Christina Rosso (the “Instructor”) during which I will receive information about and instruction to perform yoga postures. I understand that Yoga is not a substitute for medical attention, examination, diagnosis or treatment. I recognize that Yoga requires physical exertion which may be strenuous and may cause or aggravate physical injury and I am fully aware of and willingly assume the risk and hazards involved. As is the case with any form of physical activity, I understand that it is my responsibility to consult a physician regarding my ability to participate in yoga prior to participating. My signature verifies that I have either (a) been physically examined by a physician, and been given such physician’s permission to participate in these classes or (b) agreed to participate in these activities without the approval of my physician. I understand that participation in Yoga could, in some circumstances, result in abnormal blood pressure, fainting, heartbeat disorders, physical injury, heart attack, serious injury or death. As in the case with any physical activity, the risk of injury, even serious or disabling, and death is always present and cannot be entirely eliminated. I am fully aware of this risks and choose to participate at my own will. The Yoga practice and/or specific poses are not recommended and/or safe for individuals with certain medical conditions, whether preexisting or unknown, including cardiac illness, pregnancy and post-surgery and those taking certain medications or supplements. I affirm that I alone am responsible to decide whether to practice yoga and I practice at my own will. I understand that Yoga Alignment’s Yoga classes are not intended for anyone in need of supervised rehabilitation. I am responsible for knowing which movements or body positioning to avoid due to any medical condition(s), injury, medication, pregnancy, or pain and am responsible for avoiding those movements and body positions throughout the yoga classes. This Waiver Agreement shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I acknowledge that this Waiver Agreement form will be used by the persons or entities being released in the Yoga classes and that it will govern my actions and responsibilities in said classes. By signing this document, I understand that I will waive certain legal rights, including the right to sue. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors and assigns as follows: I (a) irrevocably WAIVE, RELEASE AND DISCHARGE FROM ANY AND ALL LIABILITY for my death, disability, personal injury, or actions of any kind that hereafter may occur to me, the Instructor and Yoga Alignment, who is hosting these classes and where sessions are being held, and each of their employees, volunteers, representatives or agents; and (b) INDEMNIFY, HOLD HARMLESS AND AGREE NOT TO SUE the entities or persons mentioned in this paragraph as to any and all liabilities or claims made as a result of participation in the Yoga classes, whether cause by negligence of releases or otherwise. My signature further acknowledges that I shall not now or at any time in the future bring any legal action against the Instructor and/or Yoga Alignment; and that this waiver is binding on me, my heirs, my spouse, my children, my legal representatives, my successors and my assigns. In consideration of participation in these Yoga classes with the Instructor at Yoga Alignment, I hereby release the Instructor and Yoga Alignment from any claims, demands and causes of action. I release Yoga Alignment and the Instructor from any liability now and in the future in respect to physical injuries, heart attacks, broken bones, soreness or other injury or ailment, however caused, occurring during or after my participation in this yoga or fitness program. My signature acknowledges that during Yoga classes, I will not perform any postures to the extent of strain, extreme discomfort, or pain. If at any point I feel overexertion or fatigue, I will respect my body’s limitations and I will rest before continuing the Yoga practice. I understand my physical limitations and am sufficiently self-aware to stop physical activity before I become ill or injured. I hereby certify that I have read this document carefully; and, I fully understand its contents. I am aware that this is a release of liability as well as a contract and I sign it of my own free will, voluntarily executing it without pressure from any third party. *

I CONFIRM THAT I HAVE READ THIS RELEASE AGREEMENT AND I AM AWARE THAT BY SIGNING THIS RELEASE AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS AND REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES. I ACKNOWLEDGE THAT THIS RELEASE AGREEMENT SHALL APPLY TO ALL FUTURE PARTICIPATION IN YOGA AND FITNESS ACTIVITIES
